We are working in partnership with a highly respected hospice, who provide care for adult patients, and their families, living with cancer or another terminal illness across West Surrey and part of North East Hampshire. The charity is seeking to appoint a Patient, Family and Carer Support Team Manager, to provide team leadership and management support, for the delivery of responsive high-quality care to patients with advanced illness, their carers and relatives at home, or on the In-patient Unit. The multiple discipline team includes counselling, psychology, social work, patient welfare, chaplaincy, pastoral support, and volunteer home support. The postholder will be a member of the Senior Clinical team and will lead projects relating to inclusion and access. They will also be an ambassador for partnership working within the health, social care and the voluntary sector.
The successful individual will need to demonstrate:
A qualified health care professional, for example Social Worker, Nurse, Counsellor
A breadth of experience of working at a senior level and of managing multi-professional teams in a clinical environment
Experience of budget management
Experience of service delivery
Experience of working to support inclusion, diversity and improve access to services
An understanding of end-of-life care, from personal or professional experience
We are seeking a strategic, approachable, pro-active team leader. They will be empathic to the needs and development of their multi-professional teams, with the ability to manage and lead by example. Advanced communication and interpersonal skills are vital.
